Patient Engagement at a Transitions Clinic Network Program

In a Transitions Clinic Network Program, patient engagement is enhanced when community health workers with lived experience of incarceration are embedded within health care teams. Additionally, health care teams are trained to work effectively with CHWs, to be culturally responsive, and to apply non-stigmatizing and trauma-informed strategies. A program’s relationships with community stakeholders and knowledge of local community resources allow for coordination of services and wrap-around support for patients.
